cin as well as 6 months of daily albendazole 400 mg b.i.d. for the first 3 weeks of every month. A week later he was readmitted due to recurrent syncope and hypotension. He had an elevated JVP and no breath sounds on the right side. There was also marked new eosinophilia. CT demonstrated a new massive right pleural effusion compressing the superior vena cava and right heart and necessitating emergency drainage of what was an overt empyema with a pH of 6.8 ( fig. 2 a). During hospitalization he reA 38-year-old otherwise healthy man presented to the emergency department with a 3-day history of fever, dyspnea, pleuritic chest pain, and hemoptysis. CT scan suggested a hydatid lung cyst ( fig. 1 ) without evidence of other organ involvement. Sputum microscopy revealed echinococcal protoscolices of various maturations, and sputum cultures yielded Acinetobacter baumanii. The patient refused surgery and was discharged for medical therapy with 2 weeks of oral clindamycin and levofloxaPublished online: August 4, 2012

Echinococcal tension pneumothorax.
Hydatid cyst is rarely mentioned among the causes of pneumothorax in text-books or monographs, especially those written in English. Five examples of tension pneumothorax secondary to ruptured hydatid cyst of the lung are reported: the mechanism of this tension effect and helpful diagnostic points are discussed. We think that surgical correction is the only satisfactory treatment of tension pneu...
متن کاملRuptured Hydatid Cyst with an Unusual Presentation
Ruptured pulmonary hydatid cyst may sometimes cause complications like empyema, bronchopleural fistula, and collapsed lung. These complications may mislead the diagnosis and treatment if prior evidence of cyst has not been documented before rupture. We present a case of a young male who presented with complete collapse of left lung with pyopneumothorax and bronchopleural fistula which was misdi...

Iran is recognized as an endemic area for lung hydatidosis. Surgical removal of some hydatid cysts may have setious morbid consequences. To determine the characteristics of these special cysts, a retrospective survey was carried out on patients admitted to Modarres Hospital in Tehran between 1989 and 1998. We have found that ruptured, bilateral, lung located, and giant cysts (> ] 0 cm) caus...
